The issue is on your Custom Form customization/data source, firstly you might check if the custom form template has the same data source as the Standard one used for your test. In case they are not the same, this configuration might already solve your issue. Furthermore, in case your issue persists, you might now check data binding from your custom form template by double clicking the field in Adobe Livecycle and analyzing further.
