MOC 1909 setup for PM - Questions do not appear on the CR

I am in the process of configuring MOC in 1909 and have followed the SAP Help document several time around setting up Questions. When I create and submit a CR all other Activities appear but the Questions don't. I know this is new but can anyone shed some light on what I have missed please.

Also if anyone can send me a screen shot so I can see how the questions appear on the CR that would be great because there is no information to that detail on the internet.

I have found out how to do these Questionnaires. The SAP help missed that you need to create a Question Catalog under the Questionnaire. Inside the Question Catalog you add your questions. The Questionnaire is then added to the CR type.
