To be honest it's not easy to get the exact question.
there can be various types of issues in a system, e.g. an S/4HANA system: you can have basis-topics (database topics, authorization issues, Background RFCs, Technical Users, Workflow Tasks...). - Just to name some standard transactions ST22, SM21, ST01, SU53, SLG1, SBGRFCMON and multiple more...Sometimes a note needs to be implemented or an upgrade to a newer release is required.
There could be errors in setting up the workflow (rule-based) or the validations or derivations (with custom code in BAdIs or in BRF+) if not using standard ones. Also if enhancing the data model or the Webdynpro UI in an 'invalid way' you can run into several issues.
... you can have replication issues (DRFLOG), if you are sending data to another system when e.g. the value mapping is not customized correctly or the replication is not customized correctly...
Let's say there are so many ways or reasons why an error can occur in a system (independently on MDG itself) that it is hard to answer it.
